In Australia, the journey from pregnancy to childbirth often involves multiple healthcare providers and institutions duet to the structure of the nation healthcare system. It is common for different doctors to handle prenatal care, delivery, and postnatal support. However, this fragmented care model poses challenges, especially in urgent situations where unfamiliar medical professionals must quickly assess a patient’s condition and make informed decisions.
So, how can clinicians efficiently access a pregnant individual’s complete medical history across institutions? How can they ensure continuity of care and avoid missing critical health details for both the mother and the baby?
Our team goal is to design a unified pregnancy care platform that consolidates health data across all stages of pregnancy. This platform allows various doctors and institutions to securely share and access consistent, up-to-date medical information—supporting timely, informed, and personalized maternal care.

How information is organized shaped its impact.
The biggest thing throughout this project is how information is organized shapes its impact. The same medical data whether in the form of simple numbers, text or visual charts and diagrams can lead to very different outcomes depending on how it’s structured and delivered. We found that transforming data into clear visual insights can actually positively affect users’ decision-making and comprehensive.
This project highlighted the importance of visual design, information architecture, and clarity interface dealing with critical health information. So design is about enabling trust, understanding, and action.
